Please note: Due to the specialized services we provide and the large number of referrals that SDC receives, we currently have an 8-12 month wait time for neuropsychological and psychological evaluations. The wait time for early evaluation of Autism, 42 months and under, is 4 months.

Process

To be considered for a comprehensive evaluation, your child’s primary care physician (PCP) must determine if the level of concern warrants this service. If the PCP believes the concerns fall within the scope (dual diagnosis, neuro-developmental, neurological, chronic health, and/or learning disability not responsive to school interaction) of the services provided at SDC, he/she will submit the appropriate referral and documentation for further consideration. Please remember, a referral from your child’s PCP does not guarantee that your child will be seen.

If a referral is accepted, the “triage process” will begin. The purpose of the “triage process” is to obtain as much information as possible to determine the correct services from the start. If during the “triage process” it is determined that SDC services are not the appropriate for your child, you and the referring physician will receive notification with recommendations indicating what services would best meet the needs of your child.

Once the “triage process” has been completed, you will receive a Patient Information Packet. This is required so that we can learn more about your child and be able to best meet their needs. After the completed packet has been received, the triage team will review and request additional information as needed. If your child has been previously evaluated for developmental and/or behavioral concerns (i.e., speech therapy, psychoeducational, developmental or psychiatric evaluations, etc.) and you have copies of any of their reports, please mail/fax them to our clinic along with the patient information packet.

When all information has been obtained, a clinician will review and determine if a comprehensive evaluation is warranted.  Once the need for an evaluation has been determined, an appointment can be scheduled.  Unfortunately, due to the specialized services we provide and the large number of referrals that SDC receives, families often have a waiting period for an initial appointment. Currently, our wait time is 8-12 months.  The wait time for early evaluations of Autism, 42 months and under, is 4 months.
